Talk, exposure, CBT, EMDR, SIT, medication — Invented a lot of ways to manage the horrors of our own species.

That's the problem. You test what you can to find a physical cause. If one doesn't appear, then you assume it's mental. Except you need extremely specific tests in some cases, or they have a rare illness that doctors don't think to look for, or you have to expose someone with a mental illness to a battery of stressful examinations which doesn't help their condition. People go years trying to prove their mental illness is a physical one and vice versa.
